This questionnaire should be jointly completed by parties ahead of the post-committal directions hearing (PCDH).

Use this template to make an application under s 198B of the Criminal Procedure Act 2009. 

In-person hearing form

The purpose of this form is to assist in the administrative allocation of courtrooms for in-person hearings.

Probate Office – Affidavit of testamentary capacity

This affidavit is to be used to establish the testamentary capacity of a deceased person at the time of making their will.

This form is to be used to identify an exhibit referred to within an affidavit.

This form is to be used where a party to an existing proceeding who is not legally represented appoints a solicitor to represent them.

This form is to be used where a party to an existing proceeding changes their address for service or where a different solicitor within a firm now acts for that party.

This form is to be used where a party to an existing proceeding who is legally represented appoints a different law firm to represent them.

This form is to be used where the law firm acting for a party to an existing proceeding ceases to act for that party.

This form of summons is to be used when seeking an order to rectify a clerical error in a will.
