The consent to dispensation of surety form is to be completed and signed by the person(s) giving up the protection which a surety guarantee would have provided them.
The affidavit verifying consent to dispensation of surety is to be completed and sworn/affirmed by the person who witnessed the person sign the consent to dispensation of surety form.
